Arrange Care
To arrange services, contact VNA Care Network & Hospice's Referral Center at:
800-728-1862
For TTY access, call 508-751-6926.
Referral Center staff can answer questions, arrange services, and clarify insurance coverage. A liaison nurse may be assigned to work directly with a patient's hospital, rehabilitation facility, nursing home, case manager, or physician to smooth the transition home.
Fax Referrals
Physicians and others in the health care community may refer patients via fax by completing our fax referral form (download the PDF). Faxes may be sent to 508-756-3018, 508-751-4977, or 800-599-8298. For Medicare referrals, include a completed Face-to-Face Encounter Form (download the FFE form).
Please call our Referral Center at 508-751-6860 or 800-728-1862 to confirm receipt of fax. Center staff will contact the referral source within one hour of receiving the fax if additional information is needed or arrangements need to be discussed.
For general information about services, e-mail the Referral Center at info@vnacarenetwork.org. A member of the Referral Center staff will contact you within one business day.
ASK FOR VNA CARE NETWORK & HOSPICE BY NAME
Remember to ask for VNA Care Network & Hospice by name. VNA and hospice are generic words like hospital. Just requesting the "VNA" or "hospice" could mean another agency may come to your home.
