VNA Care Network & Hospice Joins National Home Health Quality Improvement Campaign
VNA Care Network & Hospice was recently named a premier participating agency in the 2007 Home Health Quality Improvement (HHQI) National Campaign.
Premier status agencies are considered early adopters of the campaign's goal to reduce the rate of acute care hospitalization (ACH) among home health care patients in the United States. The campaign was launched by the Centers for Medicare & Medicaid Services (CMS) in conjunction with the Home Health Quality Improvement Organization Support Center.
VNA Care Network & Hospice's home health care services allow individuals to cope with illness and injury while remaining in their own homes. Services include nursing, rehabilitation therapy, home care aide support, social work, nutrition, and disease management for a variety of illnesses including diabetes and cardiac disease.
Hospice for the terminally ill is provided in patients' homes, nursing homes and other facilities, and VNA Care Network & Hospice's three residences (Chilton House, Cambridge; Tippett Home, Needham; and the Rose Monahan Hospice Home, Worcester).

The HHQI campaign is a 12-month interdisciplinary approach to improving the quality of care provided to home health patients. Participants will receive best practice intervention packages with educational materials and resources addressing top concerns in home health including fall prevention and medication management. Agencies will be able to compare their progress against state and national benchmarks based on data from CMS.
