TD Charitable Foundation Supports Telehealth

VNA Care Network & Hospice recently received a grant for $2,500 from the TD Charitable Foundation in support of the agency's telehealth program.

Telehealth is a way for VNA Care Network & Hospice’s health care professionals to know how patients are doing between in-person visits. The small unit connects to a blood pressure cuff, scale, and other devices that take measurements important to patients’ health. The results are sent over a toll-free telephone line and are monitored by VNA Care Network & Hospice staff. Changes can be made quickly to a patient’s treatment plan if vital signs fall outside the acceptable range.

"This grant from the TD Charitable Foundation will help improve the quality of life for people with congestive heart failure, chronic obstructive pulmonary disease and other chronic conditions," said Anne Zettek-Sumner, RN, M.Ed., VNA Care Network & Hospice's telehealth coordinator. "Being able to continue living at home near family and friends means so much to our patients."

Using remote monitoring to supplement traditional, in-home nursing, rehabilitation therapy and other home health care services has numerous benefits for patients, including better management of symptoms and a decrease in unplanned physician office, hospital and emergency room visits.
