Medication Management Clinics and Visits

The majority of seniors juggle five or more medications each day. The Medication Management Program can help seniors or their family caregivers maximize the benefits of seniors' prescriptions by providing information about:

  • Medications and any precautions
  • Interactions of different medications
  • Effects of over-the-counter drugs, vitamins, and herbal remedies
  • How to organize medications to make them easier to manage

… And answers to any other questions they might have.

Our Medication Management staff are also available to talk to seniors' physician or pharmacist if needed.

SERVICES

Seniors or their family caregivers may receive free support through brown bag clinics at senior centers and other community locations and medication and safety assessment home visits for chronically ill, fragile seniors.

ELIGIBILITY AND FUNDING

Medication management services are open to persons age 60 and over and their family members or other caregivers who assist with the senior's medications. The participant must also meet the residency guidelines below. The program is free thanks to a grant from HESSCO Elder Services and the Executive Office of Elder Affairs with Title III funds from the Older American's Act as well as VNA Care Network & Hospice. Confidential voluntary donations are accepted and are used to directly increase services provided under this program.

Services are open to residents of Canton, Dedham, Foxboro, Medfield, Millis, Norfolk, Norwood, Plainville, Sharon, Walpole, Westwood, and Wrentham.

Upcoming Medication Management Clinics

Seniors can register for an individual nursing consultation. Our wellness nurse will check for potential interactions, answer questions, organize medications to make taking them easier, and more. An appointment is required. Participants are asked to bring all of their medications, vitamins, and herbal remedies and their doctor's name and phone number. Clinics are scheduled for:
