When to Consider Home Health Care

It may be time to consider home health care services if you or a loved one is unable to leave home without assistance or great difficulty and answer yes to two or more of the following questions:
- Have you been diagnosed with a new illness?
- Has your existing disease become worse or have complications arisen?
- Has your medical condition declined?
- Have you started taking new medications or have your medications changed?
- Are you age 65 or older?
- Do you have difficulty managing your own care?
- Have you recently been hospitalized?
- Have you been hospitalized or visited the emergency room multiple times for heart failure or trouble breathing?
- Have you recently had surgery?
- Have you recently had joint replacement surgery?
- Are you having difficulty living safely at home because of recent falls, accidents, etc.?
- Is your caregiver unable to help you as much as they did or not available at all (because of a move or change in their life)?
