Can VNA Care Network & Hospice's home health care services help you or a loved one?

It may be time to consider home health care services if you or a loved one is unable to leave home without assistance or great difficulty and answer yes to two or more of the following questions:

Have you been diagnosed with a new illness?
Has your existing disease become worse or have complications arisen?
Has your medical condition declined?
Have you started taking new medications or have your medications changed?
Are you age 65 or older?
Do you have difficulty managing your own care?
Have you recently been hospitalized?
Have you been hospitalized or visited the emergency room multiple times for heart failure or trouble breathing?
Have you recently had surgery?
Have you recently had joint replacement surgery?
Are you having difficulty living safely at home because of recent falls, accidents, etc.?
Is your caregiver unable to help you as much as they did or not available at all (because of a move or change in their life)?
