Safe at Home Fall Prevention Program

One of every three people over age 65 falls each year — threatening their health and independence. Two thirds of all falls occur in and around the home.

The good news is that falls aren't an inevitable part of aging. Many of the risk factors that contribute to falls are easily preventable. VNA Care Network & Hospice offers Safe at Home, a fall prevention program that helps older adults keep active and healthy and remain safely in their own homes.

Safe at Home

VNA Care Network & Hospice's nurses, physical therapists, and occupational therapists identify safety risk factors that can contribute to a fall, including:

  • Medications
  • Decreased balance, muscle weakness, and difficulty walking and moving around
  • Postural hypotension, which is an unexpected drop in blood pressure causing dizziness
  • Problems with vision, hearing, congition, or numb feet
  • Environmental hazards such as unsafe walkways and stairways, poor lighting, and slippery bathtub and shower areas

Our clinicians create a plan unique to each patient's needs to reduce an individual's risk for falling.

Arranging a Safe at Home Assessment

Our Referral Center staff will work with you and your physician to determine if a Safe at Home visit will be covered by Medicare or other health insurance. Safe at Home is also available on a private pay basis.
