The majority of seniors juggle five or more medications each day. The Medication Management Program can help seniors or their family caregivers maximize the benefits of seniors' prescriptions by providing information about:

  • Medications and any precautions
  • Interactions of different medications
  • Effects of over-the-counter drugs, vitamins, and herbal remedies
  • How to organize medications to make them easier to manage

… And answers to any other questions they might have.

Our Medication Management staff are also available to talk to seniors' physician or pharmacist if needed.

SERVICES

Seniors or their family caregivers may receive free support through:

  • Group education sessions
  • "Brown bag" medication management clinics
  • Medication and safety assessment home visits for chronically ill, fragile seniors

ELIGIBILITY AND FUNDING

Medication management services are open to persons age 60 and over and their family members or other caregivers who assist with the senior's medications. The participant must also meet the residency guidelines below. The program is free thanks to grants from the Central Massachusetts Agency on Aging, HESSCO Elder Services, and the Executive Office of Elder Affairs with Title III funds from the Older American's Act as well as VNA Care Network & Hospice. Voluntary donations are used directly for program services.

Group Education and "Brown Bag" Clinics

Group education sessions and "brown bag" medication management clinics are made possible by the Central Massachusetts Agency on Aging and the Executive Office of Elder Affairs with Title III funds from the Older American's Act as well as VNA Care Network & Hospice. Group sessions and "brown bag" clinics are open to residents of Ashburnham, Ashby, Auburn, Ayer, Barre, Bellingham, Berlin, Blackstone, Bolton, Boylston, Brookfield, Charlton, Clinton, Douglas, East Brookfield, Fitchburg, Franklin, Gardner, Grafton, Groton, Hardwick, Holden, Hopedale, Hubbardston, Lancaster, Leicester, Leominster, Lunenburg, Medway, Mendon, Milford, Millbury, Millville, New Braintree, North Brookfield, Northbridge, Oakham, Oxford, Paxton, Pepperell, Princeton, Rutland, Shirley, Shrewsbury, Southbridge, Spencer, Sterling, Sturbridge, Sutton, Templeton, Townsend, Upton, Uxbridge, Warren, Webster, West Brookfield, Westminster, Winchendon, and Worcester.

Home Visits

Home visits are funded by grants from the Central Massachusetts Agency on Aging, HESSCO Elder Services, and the Executive Office of Elder Affairs with Title III funds from the Older American's Act as well as VNA Care Network & Hospice. A limited number of visits are available for chronically ill seniors living in Ashburnham, Ashby, Auburn, Ayer, Barre, Bellingham, Berlin, Blackstone, Bolton, Boylston, Brookfield, Canton, Charlton, Clinton, Dedham, Douglas, East Brookfield, Fitchburg, Foxborough, Franklin, Gardner, Grafton, Groton, Hardwick, Holden, Hopedale, Hubbardston, Lancaster, Leicester, Leominster, Lunenburg, Medfield, Medway, Mendon, Milford, Millbury, Millis, Millville, New Braintree, Norfolk, North Brookfield, Northbridge, Norwood, Oakham, Oxford, Paxton, Pepperell, Plainville, Princeton, Rutland, Sharon, Shirley, Shrewsbury, Southbridge, Spencer, Sterling, Sturbridge, Sutton, Templeton, Townsend, Upton, Uxbridge, Walpole, Warren, Webster, West Brookfield, Westminster, Westwood, Winchendon, Worcester, and Wrentham.
