The Women's Health Network and Men's Health Partnership work with Community Health Centers, including the Framingham Community Health Center and Lowell Community Health Center, and other medical providers that focus on offering comprehensive, accessible, culturally competent, and high quality care for the most vulnerable populations in Massachusetts. With an emphasis on prevention, this program supports the use of evidence-based strategies to ensure a sustainable system of primary care and medical home management.

Components of the program include:

  • Assuring preventive screenings as defined by the Massachusetts Health Quality Partners
  • Connection to primary care providers
  • Appropriate and timely access to diagnostic services
  • Care coordination, including nursing case management, client navigation, risk reduction education, chronic disease self management, and lifestyle intervention
  • Access to free or low cost treatment, when needed
  • Patient education to manage chronic disease

This program focuses on low income, uninsured and underinsured adults age 40 to 64.
